cache stampede, how do you stop that?

ran into a cache stampede situation yesterday. A popular item expired, and suddenly thousands of requests hit the origin server simultaneously. It brought things to a crawl. Weve got basic TTLs in place, but this seems like a common problem. Curious to hear how others handle this? Are you using techniques like locking around cache expirations, probabilistic early expiration, or something else entirely?
