message queues: exactly-once processing?

Hey everyone, I've been trying to get my head around exactly-once processing with message queues, especially Kafka. At-least-once is pretty easy, right? But real exactly-once processing feels like a unicorn. Have any of you actually seen or used patterns or architectures that get close to it, or is it mostly just a theory for most situations? I'd love to hear about your experiences.
